Frequently asked questions
What is Surrogacy? Who needs it?
Surrogacy involves a woman agreeing to carry a baby for someone else. After the baby is born, the birth mother gives custody and guardianship to the intended parent or parents. Surrogacy has complex legal and medical steps that must be met. It?s important to be aware of the process and seek professional advice. A woman who agrees to carry and give birth to a baby for another person is a surrogate or birth mother. Parents of a baby born through a surrogacy arrangement are known as intended or commissioning parents. Gestational surrogacy is one type of surrogacy arrangement in which surrogate has no genetic connection to the baby. IVF allows the creation of embryos from the gametes of the commissioning couple and subsequent transfer of these embryos to the uterus of a surrogate host. Surrogacy is required in case where, women who don?t have a uterus as a result of a hysterectomy (uterus removal surgery) done due to some problem in the uterus like tumor or cancer or excessive bleeding during previous childbirth, Women who are born without a functional uterus, women who have significant abnormalities in their uterus or womb including uterine scarring (Asherman?s Syndrome) due to Tuberculosis or multiple times D & C of the uterus etc. and inability to develop a thick, supportive uterine lining, Significant medical conditions that make carrying a pregnancy risky for a woman?s health (such as hypertension), women who have an illness requiring medication which can?t be stopped but is risky to the baby if continued during pregnancy and also in cases where, women who get pregnant but suffer recurrent miscarriages. Surrogacy arrangements are medically, emotionally, financially and legally complex, Consult Dr. Soma Singh for required help and a successfull journey towards parenthood. How Egg Freezing or Sperm Freezing help Cancer Patients?
Cancer treatments hamper the fertility factor in both men and women. However, various cancer treatments affect the patient differently. The damage depends upon the number of sittings the dosages of the drugs administered, while the patient?s age is yet another important factor that needs consideration. This is true for radiation as well as chemotherapy. he effects of radiation are more serious than those of chemotherapy. They depend upon the size of the radiation field, location, and the dose given. Also, removal of uterus or ovaries, as in the case of women seeking treatment of cancers of the reproductive system, does lead to zero chance of childbearing experience. However, with IVF and various fertility preservation methods available, it is now possible to preserve fertility for the future. Donor eggs, donor sperms, donor embryos, embryo freezing, egg freezing, ovarian cortex freezing are some of the options available to cancer patients to preserve their fertility. What is Micro-TESE? How it helps?
Sperm can be retrieved directly from the testis rather than from the ejaculate. These procedures are often called TESA (testicular sperm aspiration), TESE (testicular sperm extraction) or PESA (percutaneous epididymal sperm aspiration) or Micro-TESE. The most common reason to perform this is because sperm are not present in the ejaculate. This can be because the vas deferens (the tube which carries sperm from the testis) can be blocked due to a vasectomy or if the man is a carrier of the genes for cystic fibrosis. Sometimes the sperm are present in the testis in very low quantities and are not seen in the ejaculate. Sometimes sperm are produced in very low amounts in the testis and cannot be ejaculated. In these cases a technique called micro-TESE can be performed. This is performed under general anaesthetic by a surgeon (usually a urologist) using an operating microscope. A surgeon takes samples and seeks live sperm in the samples removed from the testis. This is highly skilled work which takes a long time. Sperm obtained through TESE and micro-TESE can be used directly if there?re fresh or frozen oocytes (eggs) available or sperm can be frozen. Fertilization of the eggs is by injecting the sperm directly in the eggs in a procedure called intracytoplasmic sperm injection (ICSI). The micro TESE success rate is quite favorable for men. In fact, doctors are able to find sperm approximately 60 percent of the time during microTESE procedures and helps patients to conceive and start their journey of parenthood. How Intrauterine Insemination help women getting pregnant?
There are many reasons why couples experience difficulty getting pregnant. IUI may be useful for some of them. The most common use for IUI is when no cause for the infertility is found. Infertile women sometimes take medications (by mouth or as an injection) that cause their ovaries to mature several eggs at once. The goal is to increase the chance of pregnancy by putting more sperm in contact with more eggs. IUI is helpful when a woman?s cervix has scarring that prevents the sperm from entering the uterus from the vagina. This may be seen in women who have had surgery on their cervix (cryosurgery, cone biopsy, Loop Electrosurgical Excision Procedure (LEEP). IUI can also assist when a woman has a cervix that is shaped abnormally in a way that prevents the passage of sperm. IUI can also be used for couples in which the male partner cannot become or stay erect or is not able to ejaculate effectively, or at all. For example, retrograde ejaculation is when the sperm are released backward into the bladder, instead of through the penis, at the time of male orgasm. Prior surgeries or medical conditions, such as diabetes, can cause retrograde ejaculation. Also, IUI may help if the man has an abnormal urethral opening (opening of the penis). Most of the time women who do not release an egg regularly (ovulate) can become pregnant through intercourse. Sometimes, IUI may be helpful. Also, Men may collect and freeze (cryopreserve) their sperm for future use before having a vasectomy, testicular surgery, or radiation/chemotherapy treatment for cancer. The sperm may be thawed later and used for IUI. Dr. Soma Singh, an expert in IUI procedure is available for consultation in Noida. What is Intracytoplasmic Sperm Injection Procedure ? Who should consider ICSI?
Intracytoplasmic sperm injection or ICSI is a procedure used to treat sperm-related infertility problems. This process is performed by a single sperm being harvested, and inserting it into a fully developed egg. If the egg fertilizes and develops into a good quality embryo it will be inserted into the woman?s uterus. ICSI is used for male is infertility, for men with little or no sperm present, prior failed fertilization on a IVF attempt, PGD, for patients using donor sperm with IVF, and for those with unexplained infertility that have failed to conceive with inseminations. The procedure requires that the female partner undergo ovarian stimulation with fertility medications so that several mature eggs develop. These eggs are then aspirated through the vagina using vaginal ultrasound, and incubated under precise conditions in the embryology laboratory. The semen sample is prepared by centrifuging, or spinning the sperm cells through a special medium. This separates live sperm from debris and most of the dead sperm. The embryologist then picks up the single live sperm in a glass needle and injects it directly into the egg. ICSI is considered absolutely necessary is in the case of male factor infertility with an abnormal semen analysis. Patients are electing to undergo ICSI for reasons other than male factor infertility, including previous poor fertilization with IVF, variable sperm counts and unexplained infertility. Many patients choose to undergo the ICSI procedure in order to maximize their success even when the procedure is not clearly indicated. Consult Dr. Soma Singh for an elaborative discussion about the procedure. Book an online appointment on the website or simply download icareheal app on your smart phones.
What is IVF? How it can help patients?
In vitro fertilization (IVF) is a treatment for infertility in which a person's eggs (oocytes) are fertilized by sperm in a laboratory dish. The fertilized eggs develop into embryos in the IVF laboratory. One or more of the embryos are then transferred into the woman's uterus, where it is hoped that one will implant and produce a pregnancy. IVF is usually considered by couples who have Absent or blocked fallopian tubes or Severe male factor infertility (sperm counts or sperm motility is low, or sperm needs to be extracted surgically from the testicles), or one of the reason could be Advanced reproductive age, as time to conception is critical and pregnancy rates with other therapies are low, All other causes of infertility (eg, endometriosis, ovulation disorders, unexplained infertility), if treatment with other therapies is not effective. An inherited genetic disease that they wish to avoid passing on to their child could be a reason too for a couple to choose IVF. In this case, IVF is combined with preimplantation genetic diagnosis. This means that the embryos (or in some cases, just the eggs) are tested for the disease, and only those without the disease are transferred to the uterus. Premature ovarian insufficiency, although donor eggs would be required in this case. IVF has a high rate of success in helping couples to become pregnant, Women who are considering IVF should discuss the risks, benefits, and alternatives with their health care provider and partner before treatment begins. Consult Dr. Soma Singh, an IVF expert in Noida for detailed discussion about the benefits, risks and procedure, Book an appointment through website or simply download icareheal app on your smartphones.
How to Diagnose or Understand Infertility?
Causes of infertility range from something as simple as inappropriate timing of intercourse to a structural abnormality within the uterus that requires a surgical fix. Usually, though, the reason lies somewhere between those two extremes. To confuse matters further, it?s an even split between which partner is struggling with fertility. The cause is related to female infertility a third of the time, male reproductive issues a third of the time, and both partners or unknown causes a third of the time. More common causes of infertility include Absence of ovulation (anovulation) in 20% of infertile women, Abdominal issues (i.e., adhesions, scar tissue, endometriosis) seen in 20-40% of infertile women, Abnormal sperm count in 30% of infertile men. While identifying the cause of your infertility helps us determine recommendations for therapy, none of these issues closes the door on successful fertility treatments.
